Addressing Community Needs
Touchstone uses evidence based practices and determinants of health to develop programs that support whole person health.
We tailor our services to meet individual needs and preferences of people we serve. Our staff work alongside people every step of the way to help them determine their goals and thrive in their community.
Adult Rehabilitative Mental Health Services (ARMHS)
Adult Rehabilitative Mental Health Services (ARMHS) focuses on enhancing stability, social skills, and emotional regulation. Services address budgeting, household management, and employment skills in both group and individual settings.
Integrated Community Supports (ICS)
ICS offers support and training to adults (18+) in provider-controlled settings, like apartments in multi-family buildings. Services are available up to 24 hours a day, either in the individual's home or within the community.
Care Coordination
At Touchstone, we believe everyone deserves safe, affordable housing in their preferred community. We provide supportive housing with 24-hour onsite assistance and programs to help individuals find and maintain their homes.
Intensive Residential Treatment Services (IRTS)
Intensive Residential Treatment provides 24-hour mental health services in a structured, supportive environment aimed at enhancing mental health stability and developing personal and emotional skills, self-sufficiency, and independence.
Housing Program
Supportive housing is an effective solution to community challenges, combining affordable housing with essential services. It helps individuals facing complex issues achieve stability, autonomy, and dignity while promoting long-term well-being.
Intentional Communities
The Intentional Communities Program is a peer-led initiative that fosters socialization and connection. Members meet weekly to support one another in their mental health recovery.
Minnehaha Commons
Minnehaha Commons offers supportive housing to individuals aged 55 and above who have faced homelessness. Touchstone provides personalized 24-hour on-site services that are optional to residents.
Project for Assistance in Transition from Homelessness (PATH)
The PATH (Project for Assistance in Transition from Homelessness) outreach program helps individuals experiencing homelessness who aren’t currently receiving housing services. Our mission is to provide support for accessing stable housing and essential services.
Rising Cedar Apartments
Rising Cedar Apartments in Minneapolis offers supportive housing with round-the-clock on-site services. The emphasis is on community integration, person-center goal planning, and recovery within a healing environment.
Kyle Garden Square (Coming Soon)
In partnership with Alliance Housing and Property Solutions and Services, Touchstone will serve as the on-site service provider at Kyle Garden Square, our third collaboration with this team. This project will provide 59 units of affordable housing in downtown Minneapolis.
Targeted Case Management
Adult Mental Health Targeted Case Management provides a person-centered approach to recovery. Case managers conduct assessments, develop plans, make referrals, connect individuals to mental health resources, and coordinate services to support their wellness journey.
Touchstone Connections
Touchstone Connections provides behavioral health home services. This team-based approach promotes health and wellness while offering referrals to community and social services for eligible individuals.
Anoka County Community Support Program, Opening in mid-July 2025
Anoka County Community Support Program Opening Mid-July 2025 7590 Lyric Lane, 2nd Floor, Fridley, MN Touchstone Mental Health is proud to partner with Anoka County to launch a new Community Support Program (CSP) designed to support adults living with mental illness. This welcoming drop-in center will offer psychosocial rehabilitation services that promote healing, connection, and community integration. Our CSP emphasizes arts-based programming, peer connection, and skill-building to support recovery and well-being. Members will have opportunities to: • Build meaningful relationships • Participate in creative and wellness-focused activities • Strengthen independent living skills • Access community resources and support We’re excited to open our doors in mid-July!
