Mosaic Community Support Program

Mosaic offers psychosocial rehabilitation services that promote healing, connection, and community integration. Our CSP emphasizes arts-based programming, peer connection, and skill-building to support recovery and well-being.

Members will have opportunities to:

Build meaningful relationships
Participate in creative and wellness-focused activities
Strengthen independent living skills
Access community resources and support
Access fitness equipment
Participate in employment Services through RISE

Monday- Friday 11:00am -4:00pm

Second Saturday of each month 10:00am- 2:00pm

Transportation is available upon request. Call 24 hours in advance.

Eligibility

Be 18 years of age or older
Be a resident of Anoka County
Have a diagnosed serious and persistent mental illness
