Project for Assistance in Transition from Homelessness (PATH)
Program Details
The PATH (Project for Assistance in Transition from Homelessness) outreach program is dedicated to helping individuals experiencing homelessness who are not currently receiving housing services. Our mission is to provide comprehensive support to facilitate access to stable housing and essential services. The primary services provided by PATH are outreach and case management. Other services may include screening and diagnostic assessment, community mental health support, substance use treatment, limited housing assistance, and SOARS services.
Eligibility
- To qualify for the PATH program, individuals must be experiencing homelessness and not engaged in other housing support services.
- The PATH program specifically assists individuals with serious mental illness, potential co-occurring substance use disorder. Individuals must be homelessness or at imminent risk of homelessness. We focus on serving people with complex needs who are often difficult to locate and engage.
