Older Adult Outreach
Program Details
The Older Adult Outreach Program at Touchstone offers support to individuals age 55+ who live in Hennepin County. The program supports eligible individuals to maintain independence in their home, minimize isolation, improve safety and well-being by connecting them with resources and other supports.
Eligibility
- Hennepin County resident
- 55 years of age or older
- Experiencing two or more chronic issues (social isolation, low income, physical disability, cognitive or psychological dysfunction, multiple losses)
- Not eligible for waivered services
Location Information
The Older Adult Outreach team works out of the Touchstone Mental Health Administrative building.
