Integrated Community Supports (ICS)

ICS provides support and training to adults (18+) living in a provider-controlled setting, such as an apartment in a multi-family building. Services can be delivered up to 24 hours a day, either in the person’s home or in the community. 

Serices Offered:

  • Support in Daily Living: Assistance with activities like cueing, skill maintenance, guidance, and supervision. We help residents with daily tasks and coordinating community activities. 
  • Training in Community Living: We offer skill-building and instructional services to help residents acquire, retain, and improve their ability to live independently. Training is tailored to meet each person’s specific needs. 

Resident Support Process:

  • Personalized Assessment: Each resident undergoes an assessment to identify their unique needs and goals in four key areas: community participation, health, safety and wellness, and household management. 
  • Focus on Independence: Our services emphasize promoting resident autonomy and independence. 
  • Ongoing Monitoring: Progress is regularly monitored and documented in our electronic health record system to ensure that support aligns with the resident’s evolving goals.
