Touchstone Connections is Growing!

Touchstone received a grant from the Medica Foundation to expand Touchstone Connections (formerly BHH). The grant funds a new staff position so we can serve an additional 50 people this year.
Progam staff works with individuals to develop a person-centered plan that focuses on their goals, finds community resources and services that supports their wellbeing, and connects people with primary and mental health care.

Touchstone Connections stands out from the other programs we offer because it allows people to receive services without a diagnosed mental illness. The other requirements are to be at least 18 years of age, have Medical Assistance, and are not currently receiving Targeted Case Management (TCM) or Assertive Community Treatment (ACT) services.
