Services to Find and Keep Housing

Program Overview
Touchstone assists people with their housing search and move in process. Once housed, we provide in-home support, interventions, and tenancy education, so people can maintain their housing.
Independent living support, wellness education and social activities are also available for people who are approved for waiver services.
Housing Stabilization Services
- Assists individuals with the housing search and application process
- Organizes and supports an individual in their move to housing
- Provides support on responsible tenancy and lease compliance
- Provides early intervention and advocacy to prevent eviction
- Provides coaching on conflict resolution and developing relationships with property manager and neighbors
- Assists individuals to access and maintain community services
Eligibility
- Adults with a mental illness who live in Hennepin County
- Individuals must have Medical Assistance
In-Home Supports
Touchstone offers Individualized Home Supports (IHS) for people in their own apartments and Integrated Community Supports (ICS) for people in approved ICS settings. IHS and ICS provide household management, health and wellness, adaptive skills, and connections with community opportunities.

Eligibility
- Adults with a mental illness who live in Hennepin County
- Individuals must have a CADI or BI Medical Assistance Waiver
Referrals
Referrals can be made by a Targeted Case Manager, Waiver Case Manager or Housing Services Consultant.



Intentional Communities
The Intentional Communities program brings people together who live in their own apartment and creates a ‘community’ of peers who support each other in their mental health recovery.
In-home services
- Develop household management skills
- Improve living skills
- Build community connections and socialization skills
- Improve mental health management skills
- Referrals for community resources and services
Eligibility
- Adults with a serious and persistent mental illness who live in Hennepin County
- Individuals must have Medical Assistance
- Individuals must be referred by a mental health provider
